Counselee Forms Required

These forms are required prior to the initiation of all Biblical Counseling.
They can be download and printed from your computer or provided by NWGBC&D in printed form upon request.
Please take the time to carefully read them, fill them out in their entirety and write down any questions and clarifications you may have for the counselor or the Northwest Georgia Biblical Care & Discipleship ministry.
The Personal Data Inventory is one of the most important documents to the Counselor. Please take all the time necessary to fully complete this document. This will prove to be an essential resource for the initial series of introductory counseling sessions.
